1. Eligibility:

• All Clients must receive a Good Faith Exam from our MD, Idrees Mohiuddin before receiving any treatments.
• Clients must complete a health assessment form and disclose their medical history before receiving any treatments.
• Services are not recommended for individuals with certain medical conditions (e.g., kidney disease, heart conditions) or those who are pregnant or breastfeeding without prior medical clearance.

2. Service Disclaimer:

• IV vitamin therapy and IM boosters are not intended to diagnose, treat, cure, or prevent any disease.
• Results may vary between individuals, and no specific outcomes are guaranteed.

3. Appointments and Scheduling:

• Services are offered on a concierge basis, subject to availability.
• A 24-hour notice is required for cancellations or rescheduling. Failure to provide adequate notice will result in a $50 no-show fee.

4. Membership Terms (if applicable):

• Memberships require a minimum commitment of three (3) months.
• Early cancellations within the commitment period will incur a $150 cancellation fee.
• Memberships automatically renew monthly after the initial commitment period unless canceled with a 30-day written notice.
• Membership benefits are non-transferable and do not roll over to the next month.

5. Refund Policy:

• All sales are final. No refunds will be issued for unused services, treatments, or packages.
• In cases of adverse reactions, a partial credit may be issued upon review by the medical staff.

6. Liability Waiver:

• By receiving services, clients acknowledge the risks associated with IV vitamin therapy and IM boosters and agree to release the provider from any liability arising from treatments.

7. Travel Fees:

• Currently, travel fees are waived as part of our promotional offer.
• Travel fees may be implemented in the future, and clients will be notified in advance.

8. Booster Add-Ons:

• Additional boosters can be added to IV treatments or administered via IM injection at the listed rates.

9. Conduct and Behavior:

• The provider reserves the right to refuse service to any individual exhibiting inappropriate or unsafe behavior.

10. Privacy and Confidentiality:

• All personal and medical information provided by clients will be kept confidential and used solely for treatment purposes.

11. Amendments:

• These terms and conditions are subject to change at any time. Clients will be notified of updates prior to their next appointment.
